We strongly suggest supplying this report to your agent before requesting a Questionnaire, as it may satisfy your lender’s requirements.

To request your Questionnaire, please fill out and submit the following form along with the applicable fee amount(s). The charge for the Standard* Condo Questionnaire is $150. The charge for a Lender* Condo Questionnaire is $350.

Your Condo Questionnaire will be processed and returned within 5 business days of receipt of request form and payment. Due to the high volume of requests, all questionnaires are processed in the order they are received. You may select Rush Service* for an additional fee of $50 on the following page. Please note that Rush orders are also processed in the order they are received.

*The Standard Questionnaire refers to the generally accepted Fannie Mae affidavit form as provided to us by condominium attorneys Marcus, Errico, Emmer and Brooks.

*Lender Questionnaires are reviewed by legal counsel and often result in redacted or edited questions if it poses a liability threat to the Association, and Premier's disclaimer certification will be used. If you choose to have your Lender Questionnaire completed, please reply with the file to the automated email you will receive upon submitting this form.

*Rush Service applies to processing time, not expedited mail service. All questionnaires are returned via email. Adding Rush Service to your questionnaires reduces processing time to 2 business days.
